Due to the diverse responsibilities of our Foster Parents, and in response to current e-learning practices, SOS Children’s Villages Illinois provides select individuals with in-home electronic equipment in order to perform the essential duties of their position, as well as provide proper learning tools for the children in their care.
All electronic devices and accessories are the property of SOS Children’s Villages Illinois. Upon employee termination or resignation, the device and any accessories must be returned to the Department of Finance. If the device and/or charger are not returned there will be a payroll deduction. The cost for the replacement device will be $300 and cost the charger will be $30. This will be deducted from your last payroll check if resigning or terminated, or your next check if replaced while an active employee. The Foster Parent in the home is liable for any broken or damaged electronic equipment broken or damaged in the home.
Filling out this form stands as your official agreement to this policy.
SOS Children Villages Illinois requests that employees utilize “good sense” when they are using the company provided equipment. These items are provided and to be utilized for Business purposes only and are not to be removed for any reason from the designated Village home unless transporting a device for e-learning use.
These policies and guidelines may be changed at any time. You will be given notice of any changes made after filling out this form.
