Due to the diverse responsibilities of various employees throughout the company, and in response to current “work from home” practices, SOS Children’s Villages Illinois provides select individuals with take home electronic equipment in order to perform the essential duties of their position.
All electronic devices and accessories are the property of SOS Children’s Villages Illinois. Upon employee termination or resignation, the device and any accessories must be returned to the Department of Finance. If the device and/or charger are not returned there will be a payroll deduction. The cost for the replacement device will be $300 and cost the charger will be $30. This will be deducted from your last payroll check if resigning or terminated, or your next check if replaced while an active employee.
Filling out this form stands as your official agreement to this policy.
SOS Children Villages Illinois requests that employees utilize “good sense” when they are using the company provided equipment. These items are provided and to be utilized for Business purposes only.
These policies and guidelines may be changed at any time. You will be given notice of any changes made after filling out this form.
